All certificates need original and a copy. Copies of proofs of identity must be notarized. Witness must know you (but not be related) and have a specific occupation. I got all of my and my parent's documents through vitalchek.com. Make sure you specify "long form" for the birth certificates. This may mean that you're unable to order online and you'll need to mail or fax your order form.

The total application size for me was 16 pages (17 if I included 2 proofs of address) and 2 pictures.

To get the actual application you have to call or write to the nearest Consulate General of Ireland. It's great calling and hearing the accent. The list can be found here along with a bunch of other info. The site for Ohio and Kentucky is Chicago:
