Necessary for Passport
These documents are necessary for foreign-born Irish citizens. All certificates need original and a copy. Copies of proofs of identity must be notarized. Witness must know you (but not be related) and have a specific occupation.
Documents
- Parent's birth certificate + copy
- Parent's marriage certificate + copy
- Your birth certificate + copy
- Your marriage certificate + copy
- Copy of Parent's passport or driver's license (notarized)
- Copy of your passport or driver's license (notarized)
- 2 passport style photos (signed and dated by witness)
- actual application (signed and dated by witness)
I don't know specifically what's on the application, as I haven't filled one out yet. This information is pulled from this website.
To get the actual application you have to call or write to the nearest Consulate General of Ireland. It's great calling and hearing the accent. The list can be found here along with a bunch of other info. The site for Ohio and Kentucky is Chicago:
